Passport Acceptance Facility
The Tulare Public Library provides U.S Passport Acceptance services for all DS-11 forms which includes:
- First-time passports
- Replacement of lost, stolen or damaged passports
- Renewals for applicants 15 years old and under,
- Renewals for expired passports issued more than 15 years ago.
For all other renewals please review the DS-82 form information tab to see if you qualify for Self Service.
Adults ages 16 and up
Please Bring all items listed below to your appointment:
1. Completed DS-11 Passport Application in black ink only
a. Each applicant must be present
b. *NOTE for applicants age 16 or 17 please bring one parent to your appointment
c. You can fill out the application online and print it for your appointment or pick up an application at the Tulare Public Library.
2. Original Proof of U.S. Citizenship and a photocopy
a. i.e. Certified Birth or Naturalization Certificate or a previously issued U.S. Passport
3. Valid Government issued ID and Front & back photocopy of ID
a. *NOTE for applicants age 16 or 17 if you do not have valid ID, you may use parent's ID for identification
4. One Recent 2" x 2" Passport Photo for each applicant
a. The library does not provide photo service. No eyeglasses
5. $35.00 Tulare Public Library Execution Fee per applicant (cash, credit/debit card or check)
6. Check or Money Order required for the U.S. Department of State fees
a. One per applicant. See table below for amounts
b. Applicant's name and birthday must be written on the check or money order

The Passport Card is good for entering the United States on land border crossings and sea ports of entry from Canada, Mexico, Bermuda and the Caribbean. The passport card is Real ID compliant and valid for domestic air travel.
The Passport Book is good for international travel by land, sea, or air. This passport book is Real ID compliant and valid for domestic air travel.
